5th Avenue Family Practice is a member of Prime Health. Prime Health is a group of general practitioners who aim to give their patients high quality healthcare and are dedicated to achieve better health services for the community.
PrimeHealth practices operate a recall system which allows health programmes to reach people who need them the most. Our patients will automatically and confidentially be notified if they are eligible for preventative programmes, such as cervical screening and immunisation. We aim to promote your well being as well as treat your illness.

Appointment Times
The usual appointment time is about 10-15 minutes. If you feel you require more time due to the nature of your visit to the doctor, please let us know. We appreciate that your time is precious too and every effort is made to adhere to appointment times. If you are pushed for time it is a good idea to phone us before you leave home/work to confirm that we are running to schedule. Delays can occasionally occur due to emergencies. Of course we appreciate your punctuality also. Please notify us if you wish to cancel your appointment.

Laboratory Tests
At the time your doctor requested a laboratory test she/he will advise you when the results will be ready. Most blood results take about 3 days. Some special tests may take a bit longer.

X Rays and Specialist appointments
X-rays and/or special investigations will normally be arranged at Western Bay Health (Tauranga Hospital). Alternatively these can be arranged to take place at a private facility at your own expense.

We now have a prescription line for your convenience. This is a free service. Ph 0800 235 644.

CSC and HUHC
If you are a cardholder it is important that we have a record of your card number, this means we will know exactly what fee is appropriate for your consultation. Please let us know when you receive a new card so we can update our records.

Fees & Accounts
Please pay your account at the time of consultation, we accept cash, eftpos, cheques and all major credit cards, but please note we do not give out cash on eftpos transactions. For your convenience our current fees are displayed near the reception area. An increased fee may be incurred if your consultation takes longer than usual or if special equipment and/or materials are used. If you are having difficulties paying your account or are concerned about the fees, we will be happy to discuss this with you.
